Thank you for choosing our office. We feel privileged to serve as your physicians and strive to deliver the best possible care to each patient.

If you are a new patient, please plan to arrive approximately 10-15 minutes early to ensure that your registration forms are completed and medical insurance benefits are verified. Remember to bring your photo ID and medical insurance card. Please bring your medication and allergy list and important medical records including laboratory and imaging studies results.

We update your medication list at every office visit so please inform us if there was any change made by other physicians. In order to ensure continuity of care and avoid repeating the same tests, please make sure that results of the studies ordered by other providers are faxed to our office.

It is our responsibility to discuss results of every test that we order. If you do not have a follow-up visit scheduled and you did not receive a phone call regarding your results, please contact our office.

We recommend that you contact your pharmacist for medication refills. We provide you with medication refills to last until next follow-up visit. In order to approve additional refills, compliance with follow-up recommendations will be verified.

Our physicians provide consultations and laboratory services for self-paying patients. Please contact our office for additional information.

Medical Billing

We participate with many medical insurance companies and we file claims on your behalf. We recommend that you familiarize yourself with the benefits and restrictions of your medical policy. Co-payments are due at the time of your visit.
